This form should be used to request a printed Certificate of Completion for successfully completing the requirements of a Continuing Education and Workforce Development workforce training program. All requests must be audited prior to printing. You will be able to review the status of your request after submitting form.
Note: This form is to request a printed Certificate of Completion (letter-sized document). This is not the same as a Continuing Education Unit (CEU) document (half-page folded mailer sent at the end of an indivudal course). Only use this form to request a printed Certificate of Completion.
